What if there were a self-help program to help people live productive lives while they cope with chronic health conditions? Well, there is - and it's open to adults, 60 and older, in Philadelphia, Pennsylvania at no cost to participants. It's a workshop-based program coordinated by Philadelphia Corporation for Aging. The workshops are held in group settings, in senior community centers and elsewhere around the city, and they are based upon the Stanford University Chronic Disease Self-Management Program (CDSMP).

This website will familiarize you with the program that we in Philadelphia are calling Help Yourself To HealthThis is the place where individuals with chronic diseases can sign up for a workshop held in a community setting, and where organizations interested in hosting a workshop can apply to do so.
