Q. What is this program all about?

A.  It helps participants learn self-management skills needed to help deal with the symptoms of their chronic condition.

Q. What is a chronic condition ?

A.  A disease such as diabetes, hypertension, arthritis, etc. that is long lasting, and recurring. It’s often persistent and usually lasts a lifetime in which there is rarely a cure.

Q. Who can attend a workshop ?

A. Workshops primarily are for Philadelphians 60 and older, but younger participants with a chronic condition are invited, but the focus is primarily for older adults 60+.  Caregivers, relatives, spouses, are welcome to attend along with participant. People with different health problems attend the workshop.

Q. Do I need to bring anything with me to the workshop ?

A. All workshop materials will be provided. You may want to bring a small bag to carry the materials, optional. Or any items to add to your comfort, ex. Snacks, chair cushion, etc.

Q. How long is the workshop?

A.   The workshop is held once a week on the same day at the same time for 2 ½ hours over a period of six weeks. Some sites might include an introductory zero session to collect information related to the program.

Q. Is there a cost for the workshop?

A.  The workshops are delivered at NO COST to participants and host organziations. Participants will receive a course book (Living a Healthy Life with Chronic Conditions), relaxation CD or cassette tape.

Q.  What if I miss a session?

A.  If you are unable to attend any of the sessions, please make your peer/group leader aware. You must attend at least 4 of the 6 sessions to participate in the workshop and must be present during session 1 and/or session 2.

Q.  How will this workshop help me?

A.  Participants will learn techniques and gain knowledge and tools to self-manage their chronic condition. Techniques include three process skills: action planning, disease related problem-solving and decision-making. The workshop enables participants to assume a major role in managing their chronic health conditions. Results from former participants indicate improvement in managing health symptoms with fewer hospital visits. Participants attending 4 to 6 sessions will receive a Certificate of Completion.
